Mandatory acknowledgments and safety information for all ICRI research participants

1. Research Participation, Not Product Sale

I understand that my payment constitutes paid participation in an observational research study, not a commercial product purchase.

I understand that any cannabis materials provided are research tools within an IRB-approved protocol, and I am compensating ICRI for data collection services, laboratory analysis, and research infrastructure.

I understand that ICRI does not sell cannabis products and operates exclusively as a research organization.

2. Legal Compliance

I certify that I am 21 years of age or older and will provide valid government-issued identification upon request.

I certify that I am legally authorized to possess and/or cultivate cannabis under the laws of my state/jurisdiction.

I understand that cannabis remains a Schedule III controlled substance under federal law, and I assume all legal risks associated with participation.

I understand that ICRI cannot provide legal advice and recommends consulting an attorney regarding cannabis laws in my jurisdiction.

3. Informed Consent & Research Ethics

I understand that my participation is voluntary and I may withdraw at any time without penalty.

I have read (or will read) the informed consent document and understand the risks and benefits of participation.

I understand that ICRI's research is governed by an Institutional Review Board (IRB) and follows Good Clinical Practice (GCP) standards.

I consent to the collection, storage, and analysis of my research data for scientific purposes.

4. Safety & Adverse Event Reporting

I understand that cannabis use carries potential risks, including impairment, dependence, and adverse reactions.

I agree to immediately report any adverse events, negative reactions, or safety concerns to ICRI at [email protected].

I understand that ICRI will report serious adverse events to the IRB and regulatory authorities per federal requirements.

I understand that participation does not constitute medical advice, and I should consult my healthcare providerbefore using cannabis, especially if I have pre-existing medical conditions or take prescription medications.

5. Data Privacy & Confidentiality

I understand that ICRI will protect my personal information per HIPAA standards (where applicable) and industry best practices.

I consent to the use of my de-identified data in research publications, regulatory submissions, and public databases.

I understand that anonymized research data may be shared with academic partners, regulatory agencies, and published in scientific journals.

6. No Guaranteed Outcomes

I understand that research participation does not guarantee specific cultivation results, therapeutic effects, or chemical profiles.

I understand that cannabis potency, quality, and effects vary based on genetics, environment, and individual physiology.

I understand that enrollment fees are non-refundable, as they compensate ICRI for research services, not product sales.

7. Schedule III Regulatory Context

I understand that ICRI's research supports federal Schedule III rescheduling initiatives and generates Real-World Evidence per FDA guidance.

I understand that participation contributes to scientific literature that may inform future cannabis regulations and medical applications.

I understand that ICRI collaborates with academic institutions, government agencies, and industry partners to advance cannabis science.

Potential Risks of Cannabis Use

  • Impaired coordination, judgment, and reaction time (do not drive or operate machinery)
  • Short-term memory and concentration difficulties
  • Increased heart rate and blood pressure
  • Anxiety, paranoia, or panic reactions (especially at high doses)
  • Risk of dependence with regular, long-term use
  • Potential respiratory issues with smoking
  • Interactions with prescription medications (consult your doctor)

Who Should Not Participate

  • Individuals under 21 years of age
